С любопытством прочитал 10-ку достижений в кардиологии за 2007 г.:http://www.americanheart.org/presenter. ... er=3052697. По-моему, в состав впервые вошли 4 исследования, связанные со стентами. Забавно, что попытка перейти на сайт JAMA:http://www.jama.org/ приводит на сайт производителей японских автомобилей.

Из того, что реально повлияло на мою практику, я бы отметил Кураж и новые рекомендации по профилактике инфекционного эндокардита: все равно их никто не выполнял, но теперь можно и не рекомендовать.

Про Кураж... Меня посетила вот какая мысль: в какой степени его результаты можно экстраполировать на наших больных? Поясню: уже говорилось, что в принципе коронарные больные должны быть стентированы в период нестабильности. Там где это правило выполняется работают куражные выводы. А у нас большинство нуждающихся в интервенции ее не получают, регулярно дестабилизируются, пролечатся кое-как, если не помрут, и опять по новой. Постоянно сталкиваюсь с тем, что врачам психологически сложно не рекомендовать профилактику пациенту, например, с двустворчатым аортальным клапаном или миксоматозной дегенерацией МК и МР. Мой шеф, прочитав эти рекомендации, сказал, что в условиях отечественной медицины к пациентам «With the Highest Risk of Adverse Outcome From Endocarditis» относятся абсолютно все. Я задумалась…

1. Dangers of Interrupting Warfarin Therapy Weighed
Warfarin therapy may be interrupted with low risk for as long as 5 days for minor invasive procedures, but vigilance is required, the Archives of Internal Medicine reports.
Industry-funded researchers prospectively evaluated outcomes in some 1000 patients whose warfarin therapy was interrupted, most often for 5 days or fewer and to accommodate requirements for colonoscopy or oral/dental surgery. Bridging therapy (perioperative heparin) was used in less than 10% of cases.
Seven patients (less than 1%) suffered thromboembolism within 30 days of the procedure; none had received bridging therapy. Of the 23 patients who had bleeding episodes within 30 days, nearly two-thirds had received bridging therapy.

2. Cholesterol Drug Shows No Benefit, Possible Harm
Zetia (ezetimibe) doesn't slow the progression of arterial plaque but rather may increase plaque growth, according to results from the Enhance study. Merck/Schering-Plough Pharmaceuticals says it has submitted an abstract about the trial for presentation at the American College of Cardiology in March.
The trial, performed in 720 patients with familial hypercholesterolemia, found that the mean carotid intima-media thickness increased by 0.0111 mm among patients on a combination of Zetia and Zocor (marketed as Vytorin) compared with 0.0058 mm among those on Zocor alone — a statistically nonsignificant difference, the manufacturer reports.
The New York Times emphasizes that the plaque growth rate among patients on Zetia was nearly doubled. Accordingly, the paper quotes Dr. Stephen Nissen from the Cleveland Clinic as saying that Zetia should be used only as a last resort.
