Здравствуйте.
У Кати сегодня утром стал болеть животик, обратились в поликлинику к педиатру, педиатр отправила к гастроэнтерологу, которая даже не посмотрев ребенка, отправила к хирургу, за 5 минут поставлен диагноз "спаечная болезнь" и рекомендации при болях давать но-шпу и электрофорез с чем-то на живот. Я сомневаюсь по поводу электрофореза, можно ли нам его делать? С чем электрофорез, я так и не поняла, в направлении написано -
Может живот болеть от мотилиума, он же усиливает работу кишечника?

Visual Loss after Povidone–Iodine Pleurodesis
To the Editor: Our observations suggest that high concentrations of iodate in certain povidone–iodine formulations can cause postoperative visual loss, a severe complication, when used in an off-label but widely applied manner during povidone–iodine pleurodesis. We report three cases of bilateral severe loss of vision (ranging from 20/800 vision to the perception of hand motions only) after thoracoscopic surgery involving resection of parts of one lung and instillation of 200 to 500 ml of Jodobac, a 10% povidone–iodine solution, into the thoracic cavity for disinfection and to cause scarring of the pleura for prophylaxis against pneumothorax. (For clinical details of each case, see the Supplementary Appendix, available with the full text of this letter at www.nejm.org.) Ophthalmologic examination and fluorescein angiography revealed, in the first days after surgery, a breakdown of the diffusion barrier of the retinal pigment epithelium (Figure 1A). During the next 2 months of follow-up, a granular pattern and atrophy developed in the retinal pigment epithelium (Figure 1B).
Povidone–iodine pleurodesis with the use of 20 ml of 10% povidone–iodine is thought to be safe.1 Our cases were associated with the use of Jodobac, but all available povidone–iodine formulations contain sodium or potassium iodate as a stabilizer. The toxic effects of iodate on the retinal pigment epithelium are well established, and iodate is used frequently in animal models of retinal pigment epithelium damage.2 The toxic effects of iodate on the retinal pigment epithelium have also been described in humans.3 It has been proposed that toxic plasma levels are reached after ingestion of 10 mg of iodate per kilogram of body weight.4 One analysis showed that 100 ml of Jodobac contains approximately 1.5 g of potassium iodate, which is enough to easily exceed the toxicity threshold. Other solutions, such as Braunol and Betaisadona, contain only approximately 150 mg of potassium or sodium iodate per 100 ml, but the association of their use with clinical manifestations is not known. At these concentrations, it is unlikely that the toxicity threshold can be reached with the applied amounts of povidone–iodine solutions.

Уважаемая Наталия.Электрофорез для лечения спаечной болезни не применяется в странах с цивилизованной медициной.Пока,к сожалению,не существует эффективных методов лечения спаечной болезни,кроме хирургических.В медицинской литературе имеется довольно много сообщений о серьезных побочных эффектах и осложнениях применения препаратов иода для дезинфекции кожи у детей.Тиреотоксикоз-одно из них.Это при том,что дезинфицирующие иодсодержащие препараты,применяемые в странах с цивилизованной медициной,содержат преимущественно органический иод в гораздо меньшей концентрации,чем иодид калия,применяемый в России.Вводить же иодид калия ребенку через кожу с помощью электрофореза может быть довольно рискованным и ничем не оправданным мероприятием.Natalia N писал(а):Я правильно поняла, что йодид калия может серьезно навредить? Transcutaneous Iodine Absorption in Adult Patients with Thyroid Cancer Disinfected with Povidone-Iodine at Operation

Povidone-iodine is used as disinfection in patients undergoing many kinds of operations. Several cases of thyroid dysfunction induced by transcutaneous absorption of povidone-iodine have been reported in small infants. However, transcutaneous absorption was not clearly reported in adults. The aim of this study was to assess transcutaneous absorption of iodine in patients who received single topical application with povidoneiodine and serial changes of urinary iodine excretion under the condition with a simple iodine-restricted diet in Japan, an iodine-sufficient area. Sixty-eight patients with thyroid carcinoma undergoing total thyroidectomy received single skin disinfection with either povidone-iodine (group A; n = 47) or chlohexidine gluconate, a noniodine containing biguanide (group B; n = 21). In group A, urinary iodine excretion on the first day after operation increased up to 7 times that of the preoperative value. The amounts of urinary iodine correlated positively with operating time. Increased urinary iodine, however, returned to preoperative values on the third or fifth day after operation. In group B, there was no increase in urinary iodine excretion and urinary iodine excretion was ranged from 54 to 193 µg/g of creatinine on the third day of operation. In conclusion, a large amount of povidone-iodine was absorbed through healthy skin even in adults. This may possibly interfere with scintigraphy or radioactive iodine treatment, or cause thyroid dysfunction in susceptible patients.

По этой причине препараты иода не применяются у детей для предоперационной обработки кожи.
Кстати говоря,площадь поверхности кожи,обработанной расвором органического иода (бетадином-поводином),содержащего относительно невысокое (по сравнению с КI )количества иода,для операции на щитовидной железе,совсем небольшая.
Сколько калия иодида используется для электрофореза,боюсь,не знает даже врач,проводящий процедуру.Салфетка просто обильно пропитывается иодидом калия,накладыватеся на живот,покрывается электродом и начинается "лечение".

Яна,беседуете ли Вы со своими пациентами перед проводимыми Вами роцедурами?Если да,то даете ли Вы им информацию о возможных осложнениях,побочных эффектах и альтенативах предлагаемому исследованию/лечению?Или Вы им предлагаете вначале пройти процедуру/лечение,а затем(в случае возникновения осложнений)начинать "запугивать" пациентов возможностью еще более серьезных осложнений?
В случае с "электрофорезом на живот" Наталия обратилась за советом и получила непрозрачый деликатный намек об абсурдности подобной процедуры.Поскольку намек оставался намеком,то он,вероятно, был расценен как нейтральный.В результате,Катюшка получила два сеанса "лечения".
