Differential diagnosis (before nuke scan):
1.Stress fracture (spondylolysis),spodylolisthesis
2.Infection (discitis)
3.Tumor

Spondylolysis is believed to be caused by repeated microtrauma, resulting in stress fracture of the pars interarticularis. Heredity also is believed to be a factor. Patients with spina bifida occulta have an increased risk for spondylolysis. Approximately 95% of cases of spondylolysis occur at the L5 level. Lyses can occur much less commonly at other lumbar or the thoracic levels. Involvement of multiple levels is rare. The process may be unilateral or bilateral.
Patients with bilateral pars defects can progress to spondylolisthesis. The degree of slippage of adjacent vertebral bodies varies and can progress over time.
Spondylolysis usually occurs in young athletes.
Patients with suspected spondylolysis should be evaluated initially with plain radiography, consisting of anteroposterior, lateral, and oblique views of the lumbar spine. The lateral views are most sensitive for detection of pars fractures, and the oblique views are most specific.
If plain radiographs are negative or inconclusive, further imaging may be warranted. MRI, CT, and single-photon emission computed tomography (SPECT) bone scintigraphy are used to further evaluate these patients. Currently, controversy surrounds the designation of one of these tests as most useful in the evaluation of spondylolysis.

Мальчик был не со спондилизисом, а с псевдоспондилолистезом.
"На рентгенограммах поясничного отдела позвоночника в прямой и боковой проекции определяются выраженные явления псевдоспондилолистеза, уступообразное смещение L4 кпереди, при незначительном смешении высоты нижележащего межпозвоночного диска." - из описания рентгенолога.
В чем разница между псевоспондилолистезом и просто спондилолистезом? Просвeтите.

Артем!Вам,вероятно,попала запись одного из псевдорентгенологов.Дело в том,что спондилолистеза (так же как и многого другого,что буйная фантазия псевдорентгенологов и узологов позволяет увидеть)в природе не существует.Спондилолистез-это когда один из позвонков буквально сползает с нижестоящего.Компрессионный перелом тела позвонка(см выше) может сопровождаться смещением,которое может выглядеть в том числе и как спондилолистез.Т.е. спондилолистез как и беременность,либо есть,либо его нет.Хотя псевдобеременность в природе существует.

А что, вы думаете с этим 8-летним мальчиком? Повышение температуры в течение двух суток до 39.8 C. Небольшой кашель. Tемпература 38.7C при поступлении, пульс 115, RR 20, давление 114/49, oxygen saturation 97%. Точечная боль в грудном отделе позвоночника. Движение в полном объеме.

Страшно даже подумать, но храбро зажмурившись, похоже на перелом ребра + инфекция (пневмония?)
