В детской эхокардиографии используется дополнительная позиция для диагностики и оценки размеров артериального протока. Датчик помещаем в левую подключичную область парастернально и медленно "опускаемся " вниз до визуализации протока, при этом, в большинстве случаев мы можем видеть проток на протяжении а не только часть джета в легочной артерии, следует отметить, что хорошо видно проток и в ч\б изображении. Иногда это удается и у взрослых (крайне редко). Конечно это не откровение,- так в рамках тематического разнообразия ...

Which type-> III? i think this PDA is beyond the common classification

xyz ....... This is 27 years old Filipino lady + 145cm/44kg married Norwegian. She gave birth to two babys alleredy. Last time I saw her last spring. First time by first pregnancy. She has had no time to take care of it. Her man says that they are often traveling to Philippines end ev. after this summer they can find a little more time to go to Oslo. He was surprised that Sandnessjønssykehuset have no such a med. offer and if I want they ev. can go on a journey but SSJ hospital should cover journey costs for spouse and him as a journey and hospital companion. Such is a reality

We never classify PDA as vertical horizontal
- only (size) shunt significance - what type device we will use.

12 yr old boy with no symptoms without other heart lessions.Left sided normal aortic arch.There is no final diagnosis as far.

Да проток, не маленький, но и не гигантский, увеличен ли у этой леди левый желудочек? Какой градиент сброса? При pda ,большого размера очень быстро наступает необратимая легочная гипертензия. Если ей уже 27 лет всё неплохо. можно ещё закрыть. Более печальные истории когда ребенок с 6 мм протоком в возрасте 6 лет уже неоперабелен.

Unfortunately it doesn't survive in my archive but i remeber well that TR gradient was not high

Was measured arterial pressure while Echo examination,Pawel?
Pulmonary hypertension in the course of PDA is not an absolute contraindication for occluding in adults

If we find left-to right shunt in PDA (grad >20 Hg) and LV-LA enlargement, we try to occlude it, sometimes we use testing balloon occluding. In this woman case there is huge LV and LA - it's necessary to occlude PDA
