Prof. Jarosław Kasprzak from Łódź is teaching us that echo is a good tool to evaluate dyssynchroni and there is now comeback to use echo in qualifying to CRT. I started to exercise this on our patients.
There is 77 year-old lady with congestive post-infarct cardiomyopathy with LVEDV 250ml end EF 20-25%. She was treated with ACE, diuretica, beta blocker without effect and even Entresto (sakubitril/valsartan) was tried also with no spectacular effect. She was repeatedly drained from pleura. She is coming to our department every third week on Simdax (Levosimendan) infusion.
1 year ago was implanted ICD and 6 month later was upgraded to ICD-CRT.
This is strain evaluastion. First from before CRT. Second on CRT. And third after I have changed LV-RV overtake from 30ms to 40ms…
