62 year-old man, admitted to medical award Sandnessjøenssyhehuset because of one week’s history of symptoms with cough with mixed blood sputum and fever 37,5 C. He was heavy smoker and radiologist described chest X-ray as caverna and suspicion of tuberculosis. It was Friday and CT and bronchoscopy was planned on Monday. On Monday CT lab. was broken and pulmonologist had taken only bronchoscopy. On Tuesday patient was driven by taxi to other city to take CT.
What kind of pathology patient had and wat was done with him later ?

It seems he has Gohn's focus in the right upper lobe and destruction with cavity on the left.
But considering only one week of symptoms I would begin antibiotics for lung abscess pending tests for M. tuberculosis in bronchial lavage.

It is neither TBC nor infektion...

Esophageal diverticulum? Should do barium swallow.

Barium is not a good idea, is this forum for cardiologists?

There is very difficult to give the right answer ! Right answer is in CT and with no doubt . Can you ques again.

May be, is it result of aortic disease or old trombosed pulmonary artery malformation?

This is something big and spectacular and treating effect is also spectacular

Really! Is this not result of aortic disease?!

And still don’t understand what it is. Is this healed (and sealed) pseudoaneurysm of aorta communicating with bronchus?

I think it is lung "bedsore" in result of huge aortic aneurysm
