Пациенту 50 лет при Эхо-КГ поставлена аномалия Эбштейна со стенозом трикуспидального клапана. При осмотре я сомневаюсь, что это она - так ли это? А может это отсутствие перикарда?
Пациент компенсирован. Имеется АГ 2 ст. Принимает арифон 1т. и все. АД 12-130/80 мм рт.ст. Жалоб нет.
Визуализация затруднена - четко вывести 4-х -камерную позицию сложно, да и тот кадр, который есть выводится из подмышечной области. С трикуспидальным стенозом тоже вопрос - где он. Может что- то я не досмотрела?
Аномалия Эбштейна? Отсутствие перикарда?

Стеноза ТК нет абсолютно - посмотрите какая широкая струя в систолу при ЦК (да и ПЖ не был бы расширенным, как в данном случае). Насчет аномалии ТК - пока сомневаюсь - нужны нормально выверенные позиции (хоть из подмышечной области). В отношении отсутствия (частичной агенезии) перикарда так же сомнения. Ищите причину расширения правых отделов. Ствол ЛА широкий? Другие ВПС?

Легочная артерия на уровне клапанов 2,7 см. Других пороков не вижу.
Увеличение правых отделов вполне может быть кажущимся. Строго говоря, оценивать правые отделы нужно из стандартной четырехкамерной позиции, которую в данном случае получить не удалось. Я бы не стала утверждать, что ПЖ большой.
Не смещено ли сердце влево по данным рентгенографии?
Remember that the normal cardiac position is approximately 1/3 to the right of the mediastinum and 2/3 to the left. If no heart projects to the right of the mediastinum, the appearance is abnormal. In the absence of LLL volume loss or right chest displacing pathology, the first consideration should be patient rotation. If the patient is well centred the next point of call is the thoracic shape. If the Lateral CXR demonstrates reduction of the AP dimension (straight spine) or pectus excavatum this may displace the cardiac silhouette to the left. If the chest morphology is generally normal and the heart is significantly displaced to the left and there is left cardiac border straightening consider congenital absence of the pericardium. Additional noted features may be a deep or very well defined aorto-pulmonary window caused by the absence of pleura allowing lung to invaginate into this space.
Absence of the pericardium is almost invariably on the left and is caused by incomplete formation of the pleuropericardial mebrane due to premature atrophy of the left common cardinal vein (Duct of Cuvier).The abnormality may be complete or incomplete, the latter actually being less frequent but more problematic as the PA or left atrial appendage may herniate and become incarcerated or torsed in the defect. The abnormality is more frequent in males and may be associated with ASD, PDA, Fallots, Bronchogenic Cyst, or Pectus in a third of cases.

Аномалия Эбштейна
Да с субкостальной позицией проблемы, но видно, что межпредсердная и МЖП целые. НПВ не расширена, нормально коллабирует. На ЭКГ давняя полная блокада правой ножки пучка Гиса. Из сопутствующей патологии язвенная болезнь, ремиссия.
