Читал перед сном главу о длительном кашле в Pediatric emergency medicine secrets, Philadelphia в переводе.
Самая необычная причина постоянного кашля называется - застрявшая в ухе ребёнка ресничка.
Про GER пишут, что на сегодняшний день нет надёжных подтверждений того, что постоянный кашель как-то связан с рефлюксом.

А там, пусть хоть сглатывает, хоть сплевывает - это его дело. Проблема в том, что именно этого - откашлять до полости рта - маленький ребенок физиологически еще не умеет.
Ну да ладно, тут разговор идет верю- не верю. (а для того, что бы попасть в желудок, слизи до полости рта подниматься не надо...)

( У меня новый компьютер, поэтому я не могу перегнать пока со старого - не умею, жду племянника, который придет и перегонит - информацию со старого. Там есть очень хорошая статья про Cough у детей, а, также, про вред использования муколитиков и противокашлевых у детей, подтверждающая мои неосознанные предположения:-) о том, что маленькие дети не умеют откашливать мокроту).

Объясни мне, серой американщине, что такое кашлевой рефлекс и куда (не знаю, как у вас там, не Святой Земле с чудесами) девается мокрота у неразумных чилдриков? Я-то по образованию сугубая материалистка и в меня с младых ногтей вдолбили, что материя не исчезает...
Если ребенок не может ОТКАШЛЯТЬ - не сплюнуть, а откашлять (вывести из легких в трахею->гортань->пищевод->желудок) мокроту, то тогда почему так плохи лекартства подавляющие этот самый "неэффективный" по-твоему кашель?

На самом деле, мы уже обсуждали эту тему на РМС. К сожалению, ту самую статью, о которой я говорила, четко объясняющую, почему не надо назначать маленьким муколитики при кашле, и препараты, подавляющие кашель, пока выложить не могу ( очень надеюсь, что она есть в рабочем компе, но это только завтра.):-)

Physiologically, there are similarities and significant differences between adults and children. Expectedly, the aetiologies and management of cough in a child differ to those in an adult. Я потому не поставила смаилики в моем послании, что не знала как обозначить неимоверный сарказм - было бы слишком жестко, а было бы слишком мягко.
Здоровый кашлевой рефлекс не может быть неэффективным. Точка. Любое нарушение кашлевого рефлекса несопоставимо с нормальным дренажом легких.
Мы можем выплевывать мокроту или сглатывать ее, но рефлекторно ОТКАШЛИВАТЬ мы всегда должны эффективно и за пределы респираторной системы (т.е. как минимум за надгортанник).
Иначе аспирация и каюк.
Мы можем высмаркивать сопли наружу или затягивать их в себя, но ЧИХАТЬ мы будем только в одном направлении - наружу, и причем как можно дальше.
Мы можем уметь пользоваться горшком или нет, но ОПОРОЖНЯТЬСЯ мы должны болусами и только наружу и тд, и тп. Природа! Она книжек не читала...
Возвращаясь к кашлю - именно поэтому бронхолитики хорошо, а подавляющие кашель и подсушивающие - неимоверно плохо. Пойми, что теория "неэффективного кашля" и подразумевает применение всей этой кодеиново-дексометордановой гадости.
Ну да ладно, я даже в офисе на эту тему теперь много не рассуждаю - лет 5 назад, озверев от постоянного повторения одного и того же, написала буклетик, теперь время не трачу - дите споливится и кашляет - нате оранжевый буклетик. (и тд и тп - не знаете как разобраться с жаропонижающими - нате розовенький, не так пользуете автомобильное сидение - фиолетовый, приучение к горшку по-человечески а не по-американски - желтый, кожа зимой пересохшая и раздражена - синий... тут летом не успела вовремя напечатать новые - сразу почувствовала их отсутствие...)
Так что пошла дальше отдыхать по хозяйству, а всем желаю не кашлять (особенно неэффективно)

Аллочка! Речь не идет о нарушении кашлевого рефлекса. Ну, давай вернемся к определению, что такое кашель. Кашель - это, в конце-концов, защитная реакция организма. Поэтому, подавлять кашель не нужно в принципе. ( Мы обсуждаем, разумеется банальные инфекции - ОРВИ, пневмония...и т.п.) Следовательно, вопрос о применении подавляющих и подсушивающих прeпаратах не стоит, вообще. Их применять не надо. Они, во-первых, вредны с точки зрения физиологии кашля, во-вторых, вредны с точки зрения возможных побочных эффектов, в третьих, их эффективность у детей не доказана.
Далее, препараты, разжижающие мокроту и отхаркивающие. Их эффективность не доказана, побочные эффекты имеют место быть. Уже одно это должно заставить педиатра очень хорошо подумать , прежде, чем назначать эти препараты.

New Guidelines Released for Managing Cough

HEALTH CARE WORKERS CAN NOW draw from more than 200 new recommendations from evidence-based guidelines for diagnosing and managing acute, subacute, and chronic cough thanks to a new document published by the American College of Chest Physicians. Diagnosis and Management of Cough: Evidence-Based Clinical Practice Guidelines provides recommendations for identifying and treating everything from the common cold to chronic lung conditions (Irwin RS et al. Chest. 2006;129(suppl):1S-292S).
"If physicians follow these guidelines no matter whether the patient has an acute, a subacute, or a chronic cough they've got a 90% chance of coming up with the specific cause of cough and being able to give specific treatments that will be successful," said Richard Irwin, MD, of the University of Massachusetts Medical School, in Worcester, and chair of the expert panel that wrote the guidelines.

These guidelines, which include a new emphasis on preventing whooping cough, or pertussis, in adults and new details on treating (or not treating) cough in children, will have widespread value because about 29.5 million of the estimated 829 million annual visits to office-based physicians in the United States are for cough.
TREATING CHILDREN
As reiterated in many areas of medicine, children especially young children should be treated differently than adults when it comes to respiratory conditions, including cough. Therefore, the new guidelines include a number of recommendations for treating, or not treating, children. "This is the first time we have an algorithm for cough devoted to just pediatrics," said Irwin.
Importantly, giving over-the-counter cough medications to children and adolescents aged 14 years or younger is often not a good idea, the report noted in addition to usually being ineffective in children, they can be harmful as well.
"The older antihistamines that have been shown to work in adults don't work in kids," said Irwin. In addition, while the drugs cause drowsiness in adults, they can cause children to become agitated and hyperactive.
Additional recommendations relating to children include providing prevention against pertussis infection (as part of a complete diphtheria, tetanus, acellular pertussis primary vaccination series, followed by a single pertussis vaccine booster early in adolescence); chest radiographs and spirometry (if age appropriate) for children with chronic cough; and a short trial (2 to 4 weeks) of beclomethasone or budesonide for children with nonspecific cough and risk factors for asthma.
Because these new guidelines are evidence-based, as research continues and new diagnostic tools and medications become available, additional recommendations will need to be made in the future. "The guidelines are based upon the best available evidence we have, so that means that they're going to need to be updated," said Irwin.

....За последнюю неделю , 8 детей с обструктивными синдромами, той или иной степени тяжести. Возраст от 4 месяцев до 2 лет.
Всех заставила купить небулайзеры( жалко процент с этого не имею ), веду в домашних условиях. Ингаляции с беродуалом , никаких муколитиков, один ребенок дополнительно получает дренажный массаж.
Пока у всех этих детей првый эпизод обструкции. Но один ( 4 месяца ) с атопическим дерматитом и скорее всего будем иметь детскую астму.
Пульмикорт пока не назначаю, в течение недели хорошая динамика.
Вопрос к уважаемым коллегам -педиатрам . 1-2 эпизода обструкции в год, без наличия кашля в промежутках _ назначаем базисную терапию , или нет ?
