Здравствуйте, уважаемая dr.Ira! Я тоже ужасно рад нашей новой встрече. Огромное спасибо за ссылку. Я тоже люблю этот ресурс и знаком с этим текстом. Но, к сожалению, я в нем не нашел ответов на многие свои вопросы.
Во-первых, про полезность исследования анти-H. pylori IgG мы с Вами беседовали несколько месяцев назад, когда я для себя уяснил невысокую ценность этого теста в педиатрической практике. В отношении полезности определения анти-H.pylori IgM и IgA в Feldman: Sleisenger & Fordtran's Gastrointestinal and Liver Disease, 7th ed (правда, это взрослая гастроэнтерология) читаем:
Tests for IgA or IgM antibodies are unreliable...
Ни в одних гайдлайнах мне не удалось найти указание на необходимость определения анти-H. pylori IgM и IgA с целью
диагностики хеликобактера ни у детей, ни у взрослых. Поэтому я и спросил, где об этом можно почитать.
В Nelson Textbook of Pediatrics 17th ed читаем:
Noninvasive tests include H. pylori antibody detection in serum, whole blood, urine, or saliva and antigen detection in stool. These tests have low sensitivity and specificity in low prevalence areas, especially in children. Treatment based on the results of these tests cannot be recommended.
Во-вторых, на мой взгляд, интересен вопрос: Whom to test? Там же у Нельсона читаем:
Testing for H. pylori is recommended only when there is a documented duodenal or gastric ulcer and when there is MALT lymphoma. There are no convincing data to support routine testing of children with recurrent abdominal pain or children who are asymptomatic.
В гайдлайнах от 2000 года (Helicobacter pylori Infection in Children: Recommendations for Diagnosis and Treatment), правда уже несколько устаревших и в настоящее время находящихся на ревизии Journal of Pediatric Gastroenterology and Nutrition 31:490–497 читаем:
When is testing indicated?
- It is recommended that testing be performed in children with endoscopically diagnosed, or radiographically definitive, duodenal or gastric ulcers.(Quality of evidence - I)
- It is recommended that children with recurrent abdominal pain, in the absence of documented ulcer disease, not be tested for H. pylori infection. (QoE - II)
- Testing for H. pylori infection is not recommended in asymptomatic children. (QoE - II)
- Routine screening of children with a family history of gastric cancer or recurrent peptic ulcer disease is not recommended.(QoE - II)
- Testing following treatment of documented H. pylori is recommended, especially with complicated peptic ulcer disease (i.e., bleeding, perforation, or obstruction). For patients who remain symptomatic after treatment, it is recommended that endoscopy and biopsy be performed to evaluate for the persistence of H. pylori-associated peptic ulcer disease. (QoE - I, II).
- If pathological evidence of MALT lymphoma is documented, then testing for H. pylori is recommended. (QoE - II).

Another indication is documentation of H. pylori in patients with previous ulcer disease or iron deficiency anemia.
Отсюда и был мой вопрос, где можно прочитать о поголовном обследовании каких-либо групп детей на хеликобактер, хоть адаптят, хоть кого-либо еще?
Вопрос про гастроскопию снимается - разобрался.
Ну и следующая группа вопросов сводится к следующему: Whom to treat? Несмотря на то, что вопрос о том, следует ли лечить маленьких пациентов с диагностированным наличием хеликобактера и неязвенной диспепсией продолжает дебатироваться, мне не ясны критерии по которым оценивается "меньшее из зол" - эрадикация или ее побочные эффекты (не совсем уверен, что именно это имела ввиду доктор Алла в своей фразе: "что хуже - лечение или болезнь").
