As far as I can understand, any significant vascular malformation can cause this sundrome. Hemangiomatosis is usually discovered incidentally at autopsy or surgery and is associated with a high mortality, especially in children, in whom it occurs in association with other anomalies. There are several histologic and clinical variants including: capillary, cavernous, granulation tissue type, glomus tumor and telangiectatic. There can be dissemination of the hemangiomas with cutaneous lesions and visceral involvement. Untreated, mortality is approximately 60%. The most common life threatening complications are congestive heart failure from arteriovenous shunting, consumptive coagulopathy with platelet trapping and disseminated intravascular coagulation (Kasabach-Merritt syndrome), gastrointestinal hemorrhage, and compression of adjacent structures, in particular the central nervous system. Standard conservative treatment includes steroids and interferon. More aggressive treatment options include embolization, resection or vessel ligation.
Radiology
On plain film, enlargement of the organ involved may be seen as in this case if the hemangiomatosis is gross. If symptomatic, signs of heart failure may be present. Calcifications may be seen occasionally. Sonographic appearance may be variable. Large anechoic vascular spaces within complex masses may be seen. Classically, hemangiomas are round, well-demarcated, low-attenuation lesions with peripheral nodular enhancement on CT which may fill in on the delayed images. If multiple similar lesions are seen, hemangiomatosis may be suspected. On MR, variable appearance can be seen with hemangiomatosis. Angiography is a good technique to demonstrate the blood supply and the extent of the tumor accurately as well as the magnitude of arteriovenous shunting.

11-ти летний мальчик находится под наблюдением по поводу недостаточности надпочечников. По словам родителей в последнее время он жалуется на проблемы при занятиях физкультурой ( слабость ) и при чтении. Что бы вы сделали в первую очередь:
1)определили уровень тиреоидных гормонов

3)увеличили дозу гидрокортизона
4)назначили MRI головы

Думаю, что ему надо увеличить дозу гормонов, т.к. проблема возникает при физической нагрузке.
С другой стороны, он при чтении чувствует слабость? Или стал хуже видеть? Проблема при физической нагрузке указана - слабость. Проблема при чтении - что имеется в виду? Возможно, развивается катаракта? Тогда, наоборот, надо попытаться уменьшить дозу гормонов.
Мне кажется, что для первого в жизни определения very long-chain fatty acids он уже великоват. Если бы проблема была в наличии у этого ребенка Adrenoleukodystrophy, то вряд ли бы он смог в 11 лет спортом зниматься и читать, если до сих пор был без диагноза и без лечения.

"Масло Лоренцо" ( "Шемен шель Лоренцо" ). Но ведь он к 11 годам уже не мог ни читать, ни дышать самостоятельно.
А так, конечно, при Adrenoleukodystrophy развивается и надпочечниковая недостаточность.
